Guidelines for the Anderson Family Scholarship Trust
The application must be completed, signed and submitted no later than April 1, of each calendar year. Applications are to be mailed or delivered to the Trust Department of DeWitt Bank & Trust Co., PO Box 260, 815 6th Ave., DeWitt, Iowa 52742-0260. Click here to open the application. You may complete the application on line and then print it out and mail it with all of the required attachments. You will need adobe acrobat reader to open the application.
